Debating elective single embryo transfer after in vitro fertilization: a plea for a context sensitive approach

The number of embryos transferred after in vitro fertilization (IVF) have been a topic of debate for over a decade now. Due to the risk associated with multiple pregnancy, there has been a global effort at reducing the multiple pregnancy rates to a minimum while maintaining an acceptable level of successful IVF pregnancy rate. Elective single embryo transfer (eSET) is advocated in most European countries. In Belgium and Sweden, eSET is mandatory for couples with a good prognosis. However, despite clinical recommendations and policy statements, patients in clinical practice frequently do request for the transfer of multiple embryos in order to have twins. Such requests conflict with policy guidelines and create an ethical dilemma for physicians: Should the physician do as the couple requests, and there with respect the autonomy of patients, or adhere to medical policy that takes the health of the mother and children at heart? This article provides an exploration of the arguments found in the literature that plays a role in the discussion on this topic and eventually argues that what a physician should do depends on the specificities of the context in which patients and physicians are implicated. These contextual issues can be taken into account in a shared decision‑making procedure, which allows reflections and the responsibilities of both patients and physicians to be attended in decision about assisted reproduction.Keywords: Autonomy, Embryo transfer, In vitro fertilization, Multiple pregnancy, Values and cultur

In kritisch gezelschap : richtlijnen voor een contextsensitieve morele kritiek

Hoe formuleer je een goede morele kritiek? In kritisch gezelschap onderzoekt de vraag hoe je een goede morele kritiek kunt vormen, zelfs als je er vanuit gaat dat moraliteit ontstaat in een cultuur. Die visie op de herkomst van moraal lijkt een vruchtbaar moreel debat in het dagelijks leven namelijk vaak in de weg te staan. De gangbare veronderstelling is dat mensen het onmogelijk eens kunnen worden, als moraal door cultuur wordt bepaald en mensen in Nederland verschillende culturen hebben. Ieder verdedigt toch zijn eigen sociaal-culturele achtergrond. Is die veronderstelling wel terecht? In kritisch gezelschap beweert van niet. Het eerste deel van het boek analyseert wat een moreel oordeel eigenlijk betekent, waar het over gaat, en argumenteert op basis van die analyse voor een visie op de inhoud waarop een moreel debat gericht moet zijn. De interpretatie die wordt verdedigd is geïnspireerd door Alasdair MacIntyre's visie op moraal uit After virtue, die moraal in de context van praktijken begrijpt. Deze praktische benadering lokaliseert de ontstaansgeschiedenis van moraal in concrete contexten van interactie, maar laat ook zien dat moraal uiteindelijk is gericht op de realisering van een doel, namelijk: ‘goed leven’. Deze gerichtheid op de realisering van een ‘goed leven’ zou een belangrijker rol moeten spelen in morele debatten. In het tweede deel wordt een visie op morele kritiek ontwikkeld. Als we er vanuit gaan dat moraal in culturen ontstaat, zijn er ook een paar problemen. Veel auteurs die deze visie verdedigen, zijn bijvoorbeeld conservatief: dat wil zeggen, zij hebben een sterke neiging om de moraal die in het verleden van een sociale context ontstond ongewijzigd te continueren. Hoe kun je die valkuil herkennen en verhelpen? Een ander -gerelateerd- probleem is dat auteurs die een sociaal-culturele opvatting van moraal verdedigen vaak te weinig kritisch kijken naar de rol van autoriteit in de vorming van moraal. De invloed van autoriteit is onvermijdelijk in deze benadering, want in sociale contexten oefenen altijd sommige mensen meer invloed uit op de ontwikkeling van moraal , dan andere. Maar wanneer is die autoriteit legitiem, en wanneer niet? Deze twee vragen –over conservatisme en over autoriteit- spelen de hoofdrol in deel twee en vormen het uitgangspunt voor de formulering van een visie op morele kritiek. ImmunoGlobulin galaxy (IGGalaxy) for simple determination and quantitation of immunoglobulin heavy chain rearrangements from NGS

Background: Sequence analysis of immunoglobulin heavy chain (IGH) gene rearrangements and frequency analysis is a powerful tool for studying the immune repertoire, immune responses and immune dysregulation in health and disease. The challenge is to provide user friendly, secure and reproducible analytical services that are available for both small and large laboratories which are determining VDJ repertoire using NGS technology. Results: In this study we describe ImmunoGlobulin Galaxy (IGGalaxy)- a convenient web based application for analyzing next-generation sequencing results and reporting IGH gene rearrangements for both repertoire and clonality studies. IGGalaxy has two analysis options one using the built in igBLAST algorithm and the second using output from IMGT; in either case repertoire summaries for the B-cell populations tested are available. IGGalaxy supports multi-sample and multi-replicate input analysis for both igBLAST and IMGT/HIGHV-QUEST. We demonstrate the technical validity of this platform using a standard dataset, S22, used for benchmarking the performance of antibody alignment utilities with a 99.9 % concordance with previous results. Re-analysis of NGS data from our samples of RAG-deficient patients demonstrated the validity and user friendliness of this tool. Conclusions: IGGalaxy provides clinical researchers with detailed insight into the repertoire of the B-cell population per individual sequenced and between control and pathogenic genomes. IGGalaxy was developed for 454 NGS results but is capable of analyzing alternative NGS data (e.g. Illumina, Ion Torrent). We demonstrate the use of a Galaxy virtual machine to determine the VDJ repertoire for reference data and from B-cells taken from immune deficient patients. IGGalaxy is available as a VM for download and use on a desktop PC or on a server

Asymptotic Conformal Yano--Killing Tensors for Schwarzschild Metric

The asymptotic conformal Yano--Killing tensor proposed in J. Jezierski, On the relation between metric and spin-2 formulation of linearized Einstein theory [GRG, in print (1994)] is analyzed for Schwarzschild metric and tensor equations defining this object are given. The result shows that the Schwarzschild metric (and other metrics which are asymptotically ``Schwarzschildean'' up to O(1/r^2) at spatial infinity) is among the metrics fullfilling stronger asymptotic conditions and supertranslations ambiguities disappear. It is also clear from the result that 14 asymptotic gravitational charges are well defined on the ``Schwarzschildean'' background.Comment: 8 pages, latex, no figure
